After we review your intake, you will be asked to provide the following supporting documents:

• Photo ID for all applicants, including identification for dependents (if applicable)
• Signed lease or rent report (if applicable). By submitting this, you consent to us contacting your landlord to confirm tenancy. Please ensure you include your landlord’s contact number and email address.
• Most recent utility bill (if applicable). By submitting this, you consent to us contacting the utility provider to confirm the amount owing.
• Bank statements from the last 90 days for all applicants. Please do not submit screenshots.
• Proof of Income for all applicants.

I understand that if my contact request is approved, it can take 1 - 2 weeks from the submission of my support documents before funding is dispersed.

The Basic Needs Fund (BNF) is a partnership between Distress Centre, CUPS, and Rise Calgary.

We use a secure database to store your information. By submitting this form, you consent to:

• Your information being entered into our database.
• Sharing your details with BNF partner agencies if we cannot assist you.
• Contacting your landlord or utility provider to verify amounts owed.
